Lemme guess,,, it's a wide glide front end, right? (either
dyna wide glide or softail standard)

Most likely is caused by insufficient torque on the upper
triple clamp fork stem nut (under the spin
on chrome cap)
Try taking that nut to 80 ftlbs and readjust the neck bearing
preload as needed.
Works like a charm.
